This Checklist describes all of the items that are required to be submitted in order for your loan application package to be complete. Please be advised that NIRPC is not able to review and process your loan application until all of the checklist items are received. Missing information will delay consideration of your loan request. You may submit an item as it is completed. Submit this Checklist and Attachment Form once all required items have been submitted and the squares filled in next to each item.
There are seven (7) forms that must be filled out which are listed below. They can be accessed by clicking on the form name or from the active links for these same forms that are on the Recover NWI loan program webpage, www.nirpc.org/recover-nwi. On several of these forms there are links to add the requested attachments.
